The contents of this guide are based on what I learned after attending Dave Shellnutt, The Bike Lawyer's workshop and reading their blog.
During the workshop, Dave talked about the importance of reporting bike crashes and gave us a Crash Report Card.
The card links to the Crash Report Form, and mentions the following things to write down in case of accident:
Seek medical attention via ambulance or walk in. Medical bills and other adjacent costs will likely be paid for by insurance.
